Updates from Ministry of Health (MOH). Those who plan to apply the
second statement of need to furnish MOH the following documents:
a) Offer letter from institution
b) Copy of first page of passport
c) Notarized declaration requesting for statement of need (as per format)
d) Notarized declaration -personal letter saying that you will come back to Malaysia to work
e) Cover letter from IMU
f) Curriculum vitae and also working experience in Malaysia (as per template)
g) A copy of first statement of need
h) A letter explaining 1. Explicit details on the programme that your are going to pursue
2. Contribution and benefits of the programme for you and M'sia
3. Suggested institution that you are going to work in Malaysia
once you have completed your programme/training
IMU Procedure:
a) Register as alumni
b) Fill in the request a transcript from The Alumni/Alumni Benefit/Request a Transcript
c) Provide us all the documents above.
d) We will then fax the document on your behalf to MOH.
For first time application, please provide the documents excluding document g) and h)
